Alcohol & Drugs: DUI, DWI Health Insurance w/ DUI

I have recently changed jobs and am filling out information for health insurance. It asked about DUI Conviction (misdemeanor, not yet) and whether or not I have gone through classes and had an assessment. Is this information covered by HIPAA, and my employer will not find out about this when I fill out these forms. I fear that if this gets back to them, I will be released.

Is this health insurance through your employer?

If so, is the plan fully insured or self insured? (By that I mean if you have a medical bill to be paid, does the insurance company supply the money to pay it or does your employer provide the money to pay it?) It makes a difference whether HIPAA is involved or not.

Well, you say "not yet" and you say it asks for CONVICTION as opposed to arrest, so I don't see what the problem is. The answer is no. . .
