I worked at pizza hut and they waxed the floor and did not tell me and I slipped and fell flat on my back. This happen about a month ago and my back still hurt.

Oh my, you need to get a medical exam to make sure you have no broken parts.

Pizza Hut has workers compensation insurance. Fill out as much of this form as you can, make a copy, give the original to the employer.

Go see a doctor and get checked out.
