Pizza Hut has workers compensation insurance. Fill out as much of this form as you can, make a copy, give the original to the employer.

Were there any witnesses? Did you report the incident to your supervisor at the time it happened?

 

File a WC claim & ask to be referred to an employer approved primary-care physician.
