Employer messed up my insurance

I work for a hospital that offers health insurance 30 days after hire. I took it, and my second pay check my fees started to come out. I was suppose to have all my infor by Sept. 15, but it never came. I called our hr almost 20 times and left message with No reply. I sent emails and two letters. Finally this past week, I caleld the insurance company and they had no recor dof insurance on me. Nothing. I again called my Hr office and finally talked to a real live person. She said " the paperwork was never sent cause it was missing my middle intiial. So, by this time I have paid over 580.00 into an account that isnt there. My insurance was finally started and I should recieve my paper work this week. I want that money back. It wasnt paying my insurance, what should I do to make sure I get it?
 
Talk to the US DOL. They are the regulatory agency that oversees ERISA plans, and there is every likelihood that your employer's insurance is an ERISA plan.

If the US DOL cannot or will not help, call your state insurance commission.
