Employer forged my signature on my employee evaluation

I have been employed for 15 months at my current job. April of 2008 I filled out a self-evaluation. My boss was then suppose to fill out her evaluation form for me and present it to me. I was then suppose to sign it and a copy was to go to HR, my personal file, and a copy to me. This never happened and I found out that she signed my name and sent it on to our HR dept. Is this illegal? What do I do?
 
In lieu of worrying about the legality of the issue, why don't you visit the Human Resource Department and inform them of the "error". Allow them to make the judgment.

If you start talking about legal this and legal that, you may find yourself without a job!

Thank you for your suggestion, I certainly do not want to be without a job!
Our HR dept. is out of state. Should I just email them or make a call?
