filing termination of parental rights in Wisconsin

I have form JC-1637, 11/99 Consent to Termination of Parental Rights for Wisconsin.
Could anyone tell me how to file this form and do I have to file this form before I can file form JC-1645, 11/99 Petition for Adoption _Child/Adult? Can I file them together? What happens next? I know that it is recommended to get a lawyer, but I can't afford one. If I can't get the help here I plan to go direct to the court house and ask them.

You are not going to get help over the internet on a termination of rights and adoption.

Doing this yourself is a recipe for disaster.

Are you re-married? The courts will only give you little or no information, they probably will just refer you to an attorney.

I assume you do not qualify for legal aid?

Is the person whose rights you are terminating, not disputing this?
