when is a visitation/custody/relocation case most likely to go to trial?

I am looking to relocate and am wondering what are the reasons a case would go to trial? If both parties can't agree to anything?

I believe that usually, if both parties disagree, there will be a hearing to determine the validity of the petitions. I suppose the case proceeds to trial if it can't be resolved during the hearing.
