What forms do I file?

My husband and I have custody of his two children and we live in California. We need to file a motion to modify the visitation order that we currently have. We can't afford an attorney so I am trying to do it on my own. I'm figuring I need to use forms FL-301 and FL-310. Is there a form to submit a declaration? What forms do I need to file with the court?

Also, the order also deals with child support and the child support agency is involved. Do we need to serve them a copy of the motion even though it doesn't deal with the child support issue?
