My jurisdiction is: Idaho

I am currently living with my father and am under 18. He has physical custody of me and takes care of me and my 2 sons. I don't like living with my dad, and wish to move to my moms house until I can graduate next year. Is there any way that I can just leave with her since since I am technically an adult having 2 children, or do we have to go in and switch custodies through court??

You are not an adult because you have children...As a matter of fact, that you have had TWO children before you can support them would indicate your NOT being an adult.

You are MEDICALLY emancipated, which means that you can make medical decisions for your and your children. That is all. You still don't get to decide where you live until you are 18.
