Is there a sample form that I can use to file a request to modifying the spousal support? I am 65 and just lost my job. I have a blank form from the Bucks County Law Library but would like to see what areal modification for spousal support looks like.

Otherwise, I suggest you speak with an attorney.
There might be agencies in your area that assist seniors with such request.
If not, you're on your own.
Just take a stab at it.
Its mainly common sense.
You explain to the court why a change is warranted.
Unless your former spouse is very wealthy, don't expect much to change.
Even if your former spouse is wealthy, expect him/her to have a crack team of lawyers to keep the status quo.
