Seek full custody with no residency

I had separated with my girlfriend (with who we share a daughter) and had gotten a shared(equal) custody agreement in our current state of residency. We are now both moving to Oregon,(different cities) in the next few weeks. By information that I have received from friends and family, I do not feel that she is mentally able to look after our child, much less her self. How should I apply for full custody? WHat immediate steps should I take, especially if I feel that our child's safety is at stake? and do I need to get a lawyer? I do have limited funds and do not mind doing paperwork or filing petitions for myself. She will be moving a month ahead of me so I would like to be as prepared as possible. Thank you for any assistance.

Hearsay is not admissible in court. Do you have real proof that your daughter is endangered by her mother?
 
She has physically mishandled her two older children in front of witnesses, as well as verbal abuse. The family is planning an intervention for her to receive medical attention today.

What was the result of the child protective service/police investigation when they were notified of the "mishandling"?
