I have a hand written note of my Mom's former POA and have been told by the bank that is not viable for Mom's Trust. The girl helping me said there is a form for the resignation of a POA. My question is what is the name of the form, and where can I acquire one?

There really isn't a form. A brief notarized letter of resignation is all that's needed.

However, if you have to placate some know-nothing who thinks there is a form, the following document is enough of a "form" to likely satisfy any know-nothing:
