What would cause an executor to forfeit the bond posted for an estate?

I would think that this is caused by misconduct of some sort such as a breach of fiduciary duty. The bond is posted to protect the assets of the estate. A good example is a case I worked on. An executrix moved money out of the estate without authorization. The recipient could not be found.
