Reinstating Parental Rights

My nephew's mother signed over her parental rights when he was 2 years old. He is now 5 and she is wanting to go to court to have those rights reinstated. She has been seeing him on occassion since signing over her rights. Does she have a chance in court to have her parental rights reinstated after 3 years? Any information you could give my family would be greatly appreciated. Thank you in advance.
 
If her rights were legally terminated by the courts, that is generally non reversable. Who has the child now and why have they been letting her see him/her? When rights are terminated, she is technically a legal stranger to the child so visits generally are supposed to stop. She probably has no case to get her rights back, and the visits should probably stop.
