Mom was named agent for dad in POA. She passed. I am the successor agent in POA for Dad (has dementia) but was not required to sign when POA was set up. Do I need to do anything now to legally acknowledge my new role? Will financial institutions accept my role without my notorized signature on POA?

Condolences.

Check out these NY power of attorney samples. It does not appear necessary for you to have signed anything to accept the appointment.

As for who will accept the POA, that's up to whoever you are offering it too.

All I can suggest is go about your business acting on your father's behalf. Keep a copy of your mother's death certificate attached to each copy of the POA that you use.

If you hit a snag someplace, you might have to get appointed conservator by the court.
