Name Change for Minor Child

My son and his girlfriend were separated when their daughter was born. So the mother gave the child her name. They are now together and want to change their daughter's name to her father's last name. How difficult and expensive is this?

The father will have to be established as the legal father first. He and the mother may be able to do that by signing an Affadavit of Paternity, or Declaration of Paternity.

Changing the name shouldn't be too much trouble after that. They can check at the courthouse or the office of vital statistics to get the forms and instructions.
