My jurisdiction is: New York, USA

My fiance wants to adopt my biological daughter. Him and I have been together since I was 4 mos pregnant with her. She will be turning 6 in November. I don't know who the father is. According to conception date, the guy I was with left me and I don't know anything about him. I conceived while I was in Texas. I then moved to NY. I now have two bio children with my fiance. What would it take for him to adopt her at this point? And what kind of fees are we looking at?

You will have to get married. After that wait about a year and HIRE AN ATTORNEY to help you.
 
She was born in NYC and no name is on the birth certificate, but she does have my fiance's last name who is not the bio father.

You can give her any name you like, but it doesn't make fiance legal dad. He's not. Thank goodness he didn't sign it or you could both be guilty of fraud.

As you were told, you'll need to be married at least a year, and hire an attorney, who may publish a notice in the newspaper for several weeks. You can't do this by yourself or you risk having the adoption challenged later if you missed an important step.

(And...she's your "daughter". You gave birth to her, so adding "bio" is unnecessary.)
