I live in Los Angeles, CA and have been dealing with custody issues of my 4 year old daughter ever since me and her mother broke up 2 years ago. We were never married, but finally after many months of court and legal issues we finally settled on joint legal custody having the mother with physical custody. Since then the mother has moved with her boyfriend and refuses to provide me with an address of where my daughter is now living and does not contact me regarding any schooling or health issues, per court order of having joint legal custody. In case of any emergency, I would at least like to know where my daughter is residing and be part of the decision process regarding any issues pertaining to my little girl.

I am tired of going back and forth to court, but what should I do to ensure she's abiding by our co-parenting agreement? What can happen to her legally if she keeps violating our order?

You need to file contempt NOW, that is the only way you are going to force her to cooperate. If the visitation schedule is not specific enough then make it more specific. I would see an attorney, and even ask the attorney the liklihood of you being awarded legal fees.
