How as a grandparent do I get forms and/or how do you fill these forms out?

Grandparents have "no rights" when it comes to Grandchildren if thats what you seek. You could possibly gain some rights via court but you should discuss how to proceed with an Attorney

Grandparents have ZERO rights, as regards their grandchildren.
Grandparents, under some circumstances, can petition the court for emergency guardianship of their grandchildren.
Any such emergency guardianship granted by the court, at a later could become permanent in rare circumstances.
Grandparents have no inherent LEGAL right to visit with their grandchildren.
