Full custody parent moving out of California with 3 year old

My jurisdiction is: California/San Mateo

As part of the divorce agreement, I got full legal and physical custody of my son. My son lives with me and has visitation with his father for a total of about 4 hours per week (less than stipulated in the agreement, the father lives within 20 miles but has not tried to come more often). Also as part of the agreement, I had the phrase "mother may move out of state" put in the visitation paragraph, but the family law mediation agreement says parent must get written permission to take child out of state. I want to move to Maryland with my son to go to graduate school (already got accepted and my parents live there). Do I need a special form to ask for the permission to move away or can I just type something up and get him to sign it? How do I file that with the court? If he refuses to sign it, what do I need to do? What forms do I file?
