FMLA return to work

My employee's doctor is asking her for the "FMLA return to work form"
Is there such a thing?

All I can think is they are asking for a job description and whether she is "fit for duty" and any accommodations the doctor thinks might be needed. I've never heard of a return to work form outside of something the employer requests?!?
