Alcohol & Drugs: DUI, DWI Evaluation for getting back a NY license after 2 DUIs

Convicted of second DUI in 18 month time frame 3 years ago.
Sentenced to 5 years of probation and outpatient rehab.
Got conditional license ( with ignition interlock device ) 1 year later.
Completed rehab, relapsed, completed a second Intensive Outpatient rehab last summer.
Early release from probation 2 days ago.
Stupidly smoked pot that night for the first time in 4 years.

DMV says I might need a new evaluation to get my full driving privileges back. I cannot remember if a urine screen is part of the basic evaluation or not. Should I play it safe and push the evaluation off for a few weeks while I detox, or is a urine screen never a part of the evaluation? Is the evaluation even a necessity? I can't seem to get a straight answer from anyone on that yet.

Assuming I do not mention the party a few nights ago, I've been clean for over a year, and clean for two years prior to that relapse. However, before that, I was a very heavy drinker. Is there any chance I will pass this eval or will I have to go back to rehab in order to get my full license back? Any thoughts on my situation are appreciated. Thanks.
