Some people have a close and trusted friend take care if their final wishes and desires.
You can also hire a lawyer to see to this, as well as the administration of your will and estate.
I've also seen this done by a pastor, imam, rabbi, and other religious officiants.

I used to worry about this, until I said, get busy living; nothing to worry about when I'm dead.
