Birth certificate mistake

Hey guys, I currently reside in MA but my question is relating to NY laws.

So here it goes...

My birth certificate has my mother's DOB and her last name wrong, the birth certificate states my mother's last name is the same as mine but in fact it is different. How can I go about removing my mother's "listed" last name since her real last name is listed as her middle name on my birth certificate, and how do i go about changing her DOB?

Any advice will be greatly appreciated, and if anyone knows of a good lawyer who can help me fix this I am willing to take that route too.
 
Based on what you've written, it sounds like the hospital made a mistake when filling out your birth certificate. The NYC Department of Health (and other local departments in NY), which is responsible for issuing birth certificates, has a procedure to correct mistakes (DOH form and you'd also need a letter from the hospital of birth regarding the mistake.)
