Injured and Confused

Okay here goes...my husband was injured on the job, the situation begins with him standing in front of a scrap bin. A forklift ran into him pinning him between the bin and the back of the forklift, pushing him approximatly three feet before they realized he was there. Had my husband not been there the driver would have just pushed the bin and no harm done, but he was there and was screaming for the guy to stop but for the loud machines in his area he was not freed until other coworkers came over to help him. The forklift driver was not licensed, as are most of them in this facility, my husband has been ordered to operate the forklifts without a license before. He visited the local clinic as directed by his employer that afternoon after he got off work, complaining of severe knee pain. The doctor examined him and over the next two weeks, put him in physical therapy and various tests. As the complaints continued the doctor dismissed him as just being sore and gradually lifted his restrictions until he was almost fully released. During what seemed to be one of my husband's last visits the doctor finally agreed that he should not be still in so much pain, so referred him to a specialist. At which point the specialist found that he had a torn ACL in his knee and scheduled surgery. After surgery he spent several weeks off of work and received TTD checks. Once he returned to work on restrictions he was in a department that was laid off and he was instructed by the work-comp company to file for unemployment. On January 2nd he returned to work without restrictions as they were lifted while he was laid off. He has now received paperwork from the insurance: "Agreement to compansation of employee and employer" and "medical waiver" they have asked for his signature and they would issue a settlement check in the amount of $1170.00. The rating he was issued was a 2% PPI (LE) somehow this does not seem like enough for what we could lose if he is future trouble with this knee. The doctor even told him he would probably need a total knee replacement before he is 40 due to the severety of the injury and that he should not have even been working with a torn ACL. Now that he has been released he has not returned to his old job and is being treated differently by the supervisors, should we be concerned? Should we sign the forms we received and send them back or is there anything that could result if we don't send them back right away? Should we seek legal counsel? Please help! PS: State is Indiana
