Complaints are filed with the EEOC or your state equivalent. It can depend on how many employees your employer has if & where you can file a complaint.

Not per the legal definition. Legally actionable harassment must be based on a legally protected characteristic and materially affect the terms and conditions of employment. Threats of violence would fall under criminal law, not employment law. Belittling on its own would not be actionable.
