Urgent due back today questions on claimant & employment seperation questionairres

I need help filling out these two questionnaires. How do I word it correctly & do I list the everyone's names who were there the day of separation? Thanks for your help I just don't want to ruin my chances of benefits by putting something that I shouldn't!!!!
