step-parent adoption consent papers

I have finally found my 16 year old daughters biological father and he has agreed to allow my husband of 15 years to legally adopt her, per her wishes. The question I have is: he has returned the Consent and Waiver By Parent to us signed by him and notarized, but without the 2 required witnesses. What do I need to do about this?? Must I send it back to him (he's in Idaho), or can I take care of it here in Florida?? She wants this completed before she'll even try to get a drivers license, as she wants all her official records to show her new last name.

Dad needs to actually have the document witnessed and notarized according to the law - if not, there's a chance the adoption can be overturned in the future.

Get Dad to sign another one and have the witnesses also present when the document is notarized.
