Signing over parental rights

where can you find the forms to sign over parental rights and once signed what legal actions must be taken to seal the deal

What makes you think this can be done? someone just cannot sign over your rights and wiggle out of financial responsibility. This is the case in almost all states. Unless there is a subsequent adoption, a court will not approve this.
