Permission form to move child out of state?

My daughter and I are moving to MA with my soon to be husband, where can I find the forms to get her biological father's permission? Or can it just be a signed and notarized letter from him saying we have permission?

Go to court and file a stipulated agreement modifying the current custody arrangement.

You're going to need it to modify anyway, presumably, if Dad has visitation - for example, who is going to be responsible for transportation?

You need to get that stuff ironed out before you go.

We have no court ordered agreement and neither of us want to go to court this is just more so if he changes his mind later down the line.

p.s. he has no current visitation and it seems it's in the child's best interest for him not to be apart of her life

Court orders actually PROTECT you. Because if you up and leave Dad can file IMMEDIATELY to have the child returned to California pending a custody hearing.

Can you afford to travel back and forth for that?

A notarized document means nothing, legally. It CAN show the court the parent's intent, but he's allowed to change his mind at any time.

THIS is why you need to get a court order.
