Paying for legal services.

Once a retainer is paid for retaining a lawyer's services, how is payment done? Is it a monthly payment for what's been done thus far or is there another retainer paid? Also, is there such a thing as a "Payment Plan" after racking up a large bill? Plus, what's the best way to keep tabs on what fees are being charged since child custody isn't something like arguing over a house or vehicle and costs can build quickly? Finally, will lawyers consider dropping rates down for those with meager means?
