next step please help

I adopted my step daughter in 1994 she was 3 yrs old. Her mother assured me everything was in order and I assumed the adoption was finalized. My daughter is now 16 and went to get her drivers license she could not obtain it because her social security card or birth certificate had never been changed and her mother can not provide the judgement for adoption certification because she never finished paying the lawyer that done the adoption process. How can I find out if the adoption was completed

i would first try going to the courthouse, give them the names of the parties involved and see if they have anything there as a judge has to finalize the adoption.
