Modification to Custody

Current order in place since 1996. Mother has physical Custody, Father Visitation.

In May of 2008, my teenage daughter opted to come live with me, and per a meeting with both sets of parents to discuss it, it was agreed upon that my daughter would live with me moving forward, reversing the current visitation plan. Since then I have been pursuing the mother to provide her financial information so that I can update the court papers per custody, support, OOA, etc. She has done nothing but drug her feet. Also, she has provided no support to the minor child since May 2008, and admitted to me in an email she lied about helping me do the paperwork in an effort to "punish" me.

In an effort to gets the paperwork moving forward, I went to the Self Service Center and received the paperwork to perform all the changes needed (when parties do not agree). It said I could request her financial information pursuant to ARS 25-513 and provided a form to use. I sent this information to her employer and they replied asking for what reason do I need it and to provide information stating she owes child support.

My question is Am I required to provide this information? Reviewing the ARS code suggests I need to inform them that paternity was extablished in the court documents in Sep 1996 and that I need the information to identify and locate someone for which a judgement is being sought.

Also, given her actions to "punish" me, how should I proceed? I feel she is punishing the child, not I.

Any advice would be greatly appreciated.
