I went out on family leave of medical absence due to a shoulder injury. The paperwork was handled between my doctor and the central office of the medical company I worked for. I was never told about restrictions my doctor had written. The company allowed me to work against the restrictions against my knowledge. Is that illegal?
 
No. First, there's no reason you shouldn't have talked to your doctor directly about any restrictions you might have. Second, there are only very rare instances when an employer is bound by law to follow the doctor's instructions and based on what you have posted, this is not one of them.
 
Why didn't you ask your dr. about whether you needed to work with restrictions & then brought them up/discussed them with your employer?
