Losing a child

We currently have custody of a child that is in guardianship with us. The mother could not take care of him at the time. We have had him for 2 year. 1 year with it being guardianship. He came to us at 18 months and will be 4 this year. The mother was at one time seeing him for about 6 hours a day every week. She recently has not seen him since June 27. She don't call to talk to him. We had a social worker who was for us getting him for adoption but for some reason that worker was taken off the case. Now we have a new one and the mother has requested that she wants Friday-Sunday with him. She could barely see him the 6 hours she don't call and when he comes back he is dirty and hungry. What should we do? I can't bare to loose him and send him some place where something bad could happen to him.
 
Are you related to the mother and child? You should consult an attorney about the possibility of adopting him; however, I don't think it's likely if the mother is still involved, even minimally, in his life. You can probably find an attorney who will do a free initial consultation.
