This sounds like a small claims case. Just go to the small claims court and get a form to fill out. It is extremely simple and requires virtually no guidance -- you should just state the reason for the complaint with some clarity so that the defendant can know what the complaint is about and not come to court complaining that the complaint language left him unprepared.

 

did you ever get a response to this? I could email you a damage repair estimate that we just got on the same thing...Jiffy Lube forgot to tight oil drain plug and oil filter.

 

Yes - exactly. Why not just go to your small claims court and pick up the forms. Incredibly simple to complete and designed that way.
