how to add statment of facts to petition?

When I filed the paternity suit, I used a Florida Family Law Form to do so. It gave me very little room to state my case. How do I go about attaching a separate document to the petition stating my case? Is there a certain legal title to used?

I have to prove that the mother is unfit, the evidence speaks for itself but there was no where to enter it in the petition.

Please help, I cannot afford an attorney to help me through this process.
