guardianship of adult with a disablity

My friend has a nephew that is 26 years old and disabled and receives social security every month. His mother took off (drugs) last year and left him with his younger sister. Just up and left. The older sister was able to get his checks out of the mothers name into hers. All was well, until he want to the dentist and needs a root canal on a tooth. Sister set up the appointment for the root canal, and when they got there they would not do it, because she is not his parent and he can not read or write well.
They told her she would have to get guardianship. How does she go about this? The mom is gone and can not be found. Where do they begin?
 
Visit the courthouse to get the forms and instructions for filing. Since his mother abandoned him, she should be able to get guardianship without her permission or agreement.
