I have a buddy that was arrested for drug trafficking but his charges were downgraded to possession with intent can the parole board still grid him for trafficking if that's not what he was convicted of?

The "crime severity index" in the grid is based on the most serious crime that he was actually convicted of for which he is now serving time. What do you mean by downgraded?

Note that his prior history (including charges that were reduced or dismissed after arrest) very much can count against him on the "risk to re-offend" score.
 
He was arrested for trafficking heroin but he plead out to possession with intent when they gridded him they gridded him on vgcsa opiates 14-27 which is trafficking.

Depending on where he is on the other scores, gridding him up one level on crime severity (which is all this appear to be at the worst), is good for somewhere from 2 - 18 months difference. However, understand the grid is only a recommendation. The board is free to make up their own minds as to how much time needs to be served. They're not even bound by the range in the grid.
