A member of a volunteer organization recently had a situation where a document regarding a "counseling session" was somehow released into the hands of someone who showed this document to his fiancee. Is this allowed? Does he have any legal rights to privacy?

If by counseling, you mean discipline, yes, it is allowed. I will not try to tell you that it was good management, but it did not violate any laws. There is very little right to privacy in the workplace and no laws require that disciplinary issues be kept confidential.
