I am trying to petition the court for a delayed birth certificate. For my 76 sister with cerebral palsy. My mother died in 2012 she has been in my custody since.I have been through the vital statistics and cannot get any further without the court order, I have only one of the required documents for the Dept of vital statistics to issue me a delayed birth certificate. I cannot seem to find the necessary paperwork anywhere. I did find something in Fl statutes that said I must petition the court on a form provided by the department.(Florida statutes Title XXIX public health section 382.001 short title). Help because I dont have a birth certificate I cant get her Fl ID and im not able to take her to the Dr.
