Custodial parent changed child's last name without consent

I went yesterday to pick up my son from his day care and found out that his mom (my ex) changed his last name on his day care papers from my last name to hers, which happens to be her ex-husband's last name. Legally, his last name is the same as mine (it is in our custody orders). When I confronted the child care director about this issue, she said she did not want to get involved. Can she do this without my consent? Is the day care obligated to change the info if I produce our court order? It hasn't always been like this, so she must have recently changed his paperwork at the day care center. What can I do?
 
You need to have an attorney write up a ceist and desist order and send it to her. I can understand the daycare not wanting to get involved but remind them what the last name is supposed to be. Let them know you will refer the child by the correct last name.
