child visitation modification

my husband went to court for two years and finally was able to get visitation rights.. Now his ex wife is refusing to allow him to see his child.. There are also some things that were added onto the divorce decree that we would like to change. What is the proper paperwork that needs to be filled out in order to modify the child visitation rights. We can not really afford an attorney and we are trying to do this on our own.. someone help please..

"We" can not do anything about this. In other words, the court system sees you as a legal stranger. Your husband really should consult an attorney about the visitation matter. This is not a do it yourself project.
