I don't want to answer any questions in this interrogation booklet except, if necessary; number of persons at household and first names.
Has anyone ever been fined and how do I stand legally ?
 
I can't tell you what to do but if it were me, I would stand on the Bill of Rights, more specifically, the 4th, 5th, 9th and 10th Amendments and refuse to answer any questions, not even the number of persons at household and first names. I have no idea if anyone has been fined for doing what I would do but the government cannot constitutionally tax (fine) nor prosecute anyone for exercising a constitutionally protected right.

Note, the government has the power to conduct a census but does not have the power to abrogate anyone's constitutionally protected right in order to conduct a census. How they do it is their problem.
