    Think of it this way - let's say a package arrived for you, under your real name, and I tried to pick it up. That's why they ask for ID. In your case, they have no way to know that there is no such person. As far as they are concerned it's the same as if I tried to pick up your package at the PO.

    Nothing stays on your credit report forever. 7 years is the max (other than judgments that have not been paid) and this would fall under judgment. So bankruptcy is not the answer, and it will stay on there until you take care of it.

    You need to repay the bonus. If not, they can sue you and get it back. As for the taxes, contact the IRS abou tre-filing your taxes for that year. You can go back 3 years and refile, but that needs to be done after the bonus is repaid, as you will need to show proof of repayment.
