Workers comp injury, no bill received, now it's on my credit report..

I received treatment for an injuray at work back in October 2007. It was all to be billed to my employer. In April 2009, I noticed a collection item on my credit report dating back to this injury; However, I never even received a phone call or any mail regarding an outstanding balance. I found out from the collection agency that they had been sending everything to an incorrect address and didn't even have my phon number on file, and didn't have any of my employer's information on file. My HR rep. at work said they need an itemized bill before they can pay it. I have been requesting an itemized bill for over a month and never got anything. I called the collection agency back and they said I was supposed to request the itemized bill before May 09 and they will not send me one. The balance is only $56, but I don't want to pay it because it's not MINE to pay. My main concern is having this negative item on my credit report...I want it removed whether I have to pay the balance or not.

Dispute it with the credit reporting agencies
