Wife forged signature on life insurance policy

While my wife was planning on divorcing me, she took out a life insurance policy as a benefit at her new f/t job. She forged my signature on a medical release form that sent 5 years of my medical history to an insurance company in NY without my permission. I was told this by my family doctor. I think the doctor knew it was a forgery but sent the paperwork anyways. Need to know what to do. The ex-wife admits that it is not my signature but doesn't confess to signing the document. This event, along with the recent divorce that my ex-wife petitioned, has caused me great emotion strain. Any advice would be greatly appreciated.
