US Husband died/Canadian wife getting Social Security

A Canadian family member married a US man 6-7 years ago, and the gentleman died a year after being married, she lives in Canada for 6mths and still goes to the US for the remaining 6mths. She has been recieving Social Security Benefits and Medicare since the time he died (5 years ago). She is now being told she was not elegible for these benefits. What happens now, and how could this mistake have happened?

Honestly, I don't know what happened. Mistakes do happen. Did she question why she is receiving social security benefits or did she just take the check assuming she was entitled to them? I really don't know the facts or what Medicare is demanding at this point.
