Response to a Ex Parte Restraining Order

I was served a motion for a ex parte restraining order from my ex husband and haven't been able to see or contact my kids in a week. I need to know which form to use to respond to this. I'm not getting much help from my local courthouse and I want to make sure I file the right paperwork, and need to get it in soon. Our next court apperance is on June 30th. Any help is greatly appreciated.
