power of attorney

my father has dementia and my mother, brother and myself have power of attorney over him in that order. my mother wants to give power of attorney over to my brother and myself due to health problems how do we do this? also do we have to get a letter from his doctor declaring him incompetent or can mother just give the power of attorney over to us so we can take of him?
 
In what State do you live? Usually, POA is given by the actual authorizer (your father, in this case). Is there any way you could get him to reassign the POA? If not, in certain circumstances, the 1st appointed POA can relieve the duty and reassign to the 2nd-in-line POA, as well as the 2nd POA reassigning to the 3rd POA. It really depends on the State in which you live and what's written in the original POA. Are there not any stipulations contained within the original POA that your father signed? Does it contain anything as to his 1st, 2nd and 3rd choices? I would think that it would, if it was drawn properly.
